Responsibility

As an RN in Home Health, you will play a pivotal role in providing comprehensive and personalized care to patients in the comfort of their homes. This rewarding position allows you to build strong connections with patients and their families while delivering high-quality healthcare services. Conduct thorough assessments of patients' health status in a home setting. Develop individualized care plans based on the assessment, patient needs, and physician recommendations. Administer medications and provide education on proper medication management to patients and their families. Monitor and track medication adherence, adjusting plans as needed. Wound Care and Treatment:. Perform wound care, dressing changes, and other nursing procedures as required. Ensure proper infection control measures are maintained during all patient care activities. Educate patients and their families on managing their health conditions, treatment plans, and preventive measures. Provide support and resources to promote patient and family understanding and participation in care. Collaboration and Communication:. Collaborate with physicians, therapists, and other healthcare professionals to coordinate and optimize patient care. Maintain clear and timely communication with the healthcare team and document all patient interactions accurately. Monitoring and Evaluation:. Regularly monitor patients' progress and adjust care plans accordingly. Evaluate the effectiveness of interventions and modify the plan of care as needed. Maintain accurate and detailed electronic health records (EHR) to ensure continuity of care and compliance with regulatory standards. Complete required paperwork, including progress notes, assessments, and care plans
