Executes the identified plan to ensure patients are progressing according to their personalized plan. The coordination of care aims to minimize fragmentation and duplication of services. Health teaching and promotion rely on a variety of approaches to encourage healthy practices and reduce risks. The advanced practice registered nurse and the nursing role specialist are experts in providing consultation services to influence the identified plan, enhance the competencies of others, and effect change. Evaluating the effectiveness of the methods employed in achieving outcomes. Demonstrates commitment to excellence in nursing practice through adherence to the ANA Standards of Professional Performance (2010):. Collaboration with patients, their family members, and support systems plays a crucial role in creating positive health outcomes and improving the patient experience.
