The RN Medical - Home Health role is a critical position focused on delivering high-quality, patient-centered care in the home setting. As an RN in Home Health, you will be responsible for assessing, planning, and implementing individualized care plans for patients who are recovering or managing chronic conditions in their own homes. You will work closely with physicians, family members, and other healthcare professionals to ensure patients receive the necessary care and support for their recovery or ongoing health management. In this role, you will be responsible for conducting patient assessments, monitoring vital signs, administering medications, managing wound care, and providing education to patients and their families on disease management, medication adherence, and preventative care. You will also collaborate with interdisciplinary teams to coordinate care and develop tailored plans that address the unique needs of each patient. As an RN Medical in Home Health, you will utilize your clinical skills and critical thinking to ensure the safety and well-being of patients, making timely decisions and interventions as needed. To excel in this position, candidates must hold a valid Registered Nurse (RN) license and have experience in Home Health or a similar field of nursing. Strong communication skills, the ability to work independently, and a compassionate approach to patient care are essential. A valid driver's license and reliable transportation are also required, as you will be traveling between patient homes to provide care. Required qualifications include: • Graduation from an accredited nursing program with an active RN license. • Previous experience in Home Health nursing or similar nursing experience is preferred. • Ability to work independently, manage time effectively, and prioritize tasks. • Compassionate, patient-centered approach to care, with strong interpersonal skills. • Proficiency in wound care, medication management, and patient assessments. • Strong communication skills for effective interaction with patients, families, and healthcare teams. • Valid driver's license and access to reliable transportation.
