Executes the identified plan to ensure patients are progressing according to their personalized plan. Care coordination ensures that patients receive comprehensive and continuous care. Health teaching and promotion rely on a variety of approaches to encourage healthy practices and reduce risks. To make a significant impact on the identified plan, enhance the abilities of others, and bring about change, the advanced practice registered nurse and the nursing role specialist provide consultation services. Determining the degree to which outcomes have been achieved. Applies the ANA Standards of Professional Performance (2010) to ensure safe and effective nursing practice in all settings:. Collaborates with others to develop training programs and workshops for professional growth.
