We are seeking a dedicated Nursing Services Provider specializing in Home Health to join our compassionate team. Key responsibilities for the Nursing Services Provider include: • Assessing patients' health status by conducting thorough physical exams and reviewing medical histories. • Administering prescribed medications, treatments, and therapies as per physician orders. • Monitoring patients' progress and updating care plans accordingly. • Educating patients and their families on managing chronic conditions and post-treatment care. • Providing wound care, injections, and other necessary nursing procedures. • Coordinating with multidisciplinary teams to ensure comprehensive care. • Responding to patient concerns and providing emotional support. • Ensuring that home health care services comply with relevant regulations and standards. Qualifications and Requirements: • Registered Nurse (RN) with a valid state nursing license. • Minimum of 2 years of clinical nursing experience, preferably in home health or community settings. • Ability to work independently and make critical decisions in a home health environment. • Strong communication skills to effectively interact with patients, families, and healthcare teams. • Proficiency in using electronic health records (EHR) and other healthcare software. • Valid driver's license and reliable transportation to travel to patient homes. • CPR certification and other relevant health certifications. • Ability to maintain patient confidentiality and adhere to ethical standards. • Experience with wound care, IV therapy, and chronic disease management is preferred. • Excellent time management and organizational skills.
