As a Nurse Liaison, you will foster the relationship between patients and the facilities to identify the next level of care. You will coordinate care within the Twin Falls area. Full Time Days (Mon-Fri 8 hour shifts) with rotating weekends. Works autonomously and with multidisciplinary team.
