Responsibility
LVN is responsible for providing comprehensive care in the patient's home, including the completion of skilled documentation through KanTime. Comfortable treating wounds. Serve as the patient's advocate and maintain ongoing communication with physician, referral sources, caregivers and facility management to facilitate coordination of care. Administer medications and treatments as prescribed by physician. Provides direct patient care including infusion therapy, wound care, teaching, disease management. Participate in regular case conference and in-services
