Responsibility
This position, under direct supervision of the district health director, is responsible for independently identifying and contacting persons known, suspected of transmitting, or exposed to communicable diseases such as syphilis, HIV, gonorrhea, viral hepatitis (VH) and other conditions in a culturally sensitive manner. Routine duties may include: confidentially locating and notifying persons of their positive test results, exposure and health risks; providing client-centered counseling and education; referring at risk persons for testing, clinical evaluation, treatment and other services available both on-site and in the community; collaborating with physicians and health care providers to refer and/or obtain information about patients; and interviewing infected and non- infected patients to identify partners and others (e.g., social contacts) who may be associated with the infection to prevent further transmission and adverse health outcomes. The position also provides coordinated care to to ensure that all partners and social contacts, exposed or at risk, receive adequate clinical management (testing, evaluation, and treatment), counseling and education on strategies for behavior modification to reduce health risks, and facilitate access to other needed services. This position may work outside of business hours in order to reach clients at a time and place that works for them. Performs day-to-day program oversight in a district or as part of a statewide program. Characteristic duties include: conducting studies and research, technical training and programmatic assistance, program evaluation, policy development and analysis, and coordination of resources. Monitors grant applications and disbursements to assure sufficient funds are available to support program commitments. Responsibilities are carried out independently based on an outline or work plan developed by or approved by the supervisor. Ability to work as part of a team, respond to inquiries in a professional and timely manner; interpret and apply policies and procedures;. and prepare reports and correspondence. This is a grant-funded position which is solely funded by the Office of Epidemiology for the duration of the program. The availability of funding is scheduled for review before the grant expiration date of December of each year and yearly thereafter.
