Responsibility

As a Home Health Registered Nurse, you will play a crucial role in providing comprehensive healthcare services to our patients within the familiar and supportive environment of their homes. You will work closely with patients to assess their health status, develop personalized care plans, and implement nursing interventions to promote recovery, manage chronic conditions, and enhance overall well-being. Conduct thorough assessments of patients' health conditions to determine healthcare needs. Develop and implement individualized care plans in collaboration with the healthcare team. Administer medications, perform wound care, and provide other skilled nursing interventions as needed. Educate patients and their families on managing health conditions, medications, and preventive measures. Monitor and document patients' progress, adjusting care plans as necessary. Collaborate with other healthcare professionals, including physicians, physical therapists, and social workers, to ensure comprehensive patient care. Maintain accurate and up-to-date patient records. Communicate effectively with patients, families, and caregivers to ensure a positive and supportive healthcare environment

Position Overview: As a Home Health Registered Nurse, you will play a crucial role in providing comprehensive healthcare services to our patients within the familiar and supportive environment of their homes. You will work closely with patients to assess their health status, develop personalized care plans, and implement nursing interventions to promote recovery, manage chronic conditions, and enhance overall well-being. As this is a PRN position, you will be working with us on an as-needed basis . We offer flexible scheduling, and you can freely choose the areas you prefer to cover.
