Benefits: • Competitive Compensation • Quarterly Bonuses • Company Car (for full-time field staff) • Healthcare, Dental, and Vision Plans • Life Insurance Policy • Paid Time Off • Ask about Sign-On Bonus of up to $10,000 for well-qualified individuals! • 401K Responsibilities: As a Home Health Registered Nurse, you will: • Conduct admission, case management, and follow-up skilled nursing visits for home health patients. • Administer ongoing care and case management, following directions from the Clinical Manager/CCO. • Coordinate appropriate care, collaborating with various healthcare professionals (e.g., Physical Therapists, Occupational Therapists, Home Health Aides, and external providers). • Provide regular updates on patient care, condition, and progress to the patient's physician and Clinical Manager/CCO. • Work with patients and families to implement care plans, aiming for optimal resolution of needs and functional improvement. • Supervise Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing clear written care instructions reflecting the current plan of care. • Facilitate patient discharges after consulting with the physician and Clinical Manager/CCO, ensuring all necessary clinical documentation is prepared and completed.
