We are seeking a compassionate and skilled Health Nurse to join our home health care team. As a Health Nurse in a home health setting, you will provide essential nursing care to patients in the comfort of their own homes. Your responsibilities will include assessing patients' health needs, administering treatments, and ensuring that their home health needs are met in a safe and supportive environment. In this role, the Health Nurse will be responsible for creating personalized care plans, monitoring patient progress, and collaborating with other healthcare professionals to deliver high-quality home health services. You will work closely with patients and their families, offering education on managing health conditions, administering medications, and providing emotional support. As a Health Nurse in home health, you will be required to assess and monitor the patient's physical and emotional well-being. This may involve conducting routine home health assessments, collecting vital signs, evaluating lab results, and adjusting care plans as needed. You will also be responsible for providing wound care, administering injections, and ensuring patient compliance with prescribed therapies. Key responsibilities for this Health Nurse - home health position include: • Assessing and evaluating patient needs within the home health setting. • Developing, implementing, and updating home health care plans tailored to each patient's needs. • Administering medication and treatments as prescribed, and educating patients on proper medication management. • Providing wound care, infection control measures, and other nursing services as required in the home health setting. • Collaborating with doctors, physical therapists, and other healthcare professionals to ensure comprehensive home health care. • Offering emotional support and counseling to patients and their families, while promoting independence and well-being. • Documenting patient progress, health status, and any changes in the care plan. Requirements and Qualifications: • Registered Nurse (RN) with an active and unrestricted license. • A minimum of 2 years of experience as a registered nurse, preferably in home health care or a similar setting. • Experience in patient assessments, wound care, and administering medications within a home health context. • Strong communication and interpersonal skills to work effectively with patients and their families. • Ability to work independently and manage multiple cases in a home health setting. • Excellent organizational skills and attention to detail to ensure accurate documentation and patient care. • A valid driver's license and reliable transportation are required for visiting patients in their homes. • Basic Life Support (BLS) certification is required; additional certifications in home health nursing or related fields are a plus. • Strong problem-solving skills and the ability to adapt to changing patient needs in the home health environment.
