Responsibility

This position works under the direction of the Manager of Community Health with matrix reporting to the Manager of Population Health. Responsibility includes planning, coordinating, and implementing community outreach initiatives, in target communities based on health systems strategic initiatives and priorities and needs identified through completion of Community health Needs Assessment (CHNA). This includes promoting, maintaining, and improving individual and community health by assisting individuals and communities to adopt health behaviors. The Specialist is also responsible for collecting and analyzing data to identify community needs prior to planning, implementing, monitoring, and evaluating programs designed to encourage health lifestyles. The Specialist will also effectively manage various collaborative care and value based initiatives throughout the Health System. The position will assist with the implementation of and ongoing training and operations of programs such as Blue Cross Blue Shield of Michigans (BCBSM) Patient Centered Medical Home (PCMH) and PCMH-Neighbor and Accountable Care Organization (ACO) models. This includes working with providers, staff, and administrators to provide education and leadership to maximize program value. Other duties include supporting Social Drivers of Health programs and connections with patients and community organizations through completion of the Community Health Worker (CHW) certification. (25%)*Effectively plans, coordinates and implements community outreach programs, projects and screenings that support initiatives identified in the Community Health Needs Assessment Plan. (25%) Demonstrates expertise in review and interpretation of population health data and information, and knowledge of health care and public health concepts as applied to community health needs assessments, goalsetting and plans. (25%) Demonstrate competence in project implementation with regards to value based programs like PCMH, PMCH-Neighbor and ACO. Maintain positive working relationships with key stakeholders (providers, staff, administrators, and support agencies in the community). (15%) Demonstrates skill in partnership development on community and population health initiatives and in establishing rapport with community organizations, providers, staff, and administrators. (10%) Interaction with physician offices for Patient Centered Medical Home (or PCMH-Neighbor) coaching/support. Association and IRS990 Schedule, Community Health Needs Assessment and other community benefit records. Coordinates evaluation and monitoring of community education initiatives to determine effectiveness and help with future planning. Actively participates in regular community education planning meetings, and provides monthly updates on community outreach activities. Represents MyMichigan Health on various community boards and committees that focus on health improvement initiatives and programs in the counties served. Responsible for the Community Outreach budget for the purpose of allocating funds for Community Education and Improvement initiatives. May serve as a presenter/educator for Speaker's Bureau, community organizations, businesses or professional clubs. Performs other duties that may include overseeing and tracking community sponsorships, assist in research for grant funding opportunities as well as writing efforts for community outreach initiatives across the health system;. Manages process to evaluate and analyze data for pay-for-performance criteria and other quality measures. Based on the results of the assessment, an employee may be required to participate in further learning opportunities offered by MidMichigan Health.
