We are seeking a compassionate and skilled Clinical Nurse to join our Home Health team. The Clinical Nurse in Home Health will provide high-quality patient care in the comfort of patients' homes, ensuring that they receive the medical attention they need to improve their health and well-being. This role requires a dedicated, knowledgeable, and empathetic individual who can work independently while maintaining a high standard of nursing care. As a Clinical Nurse - Home Health, you will be responsible for assessing, planning, implementing, and evaluating the care of patients with diverse healthcare needs in a home setting. You will collaborate with other healthcare professionals, including physicians, physical therapists, and social workers, to ensure comprehensive care is provided. Your clinical skills will be crucial in managing chronic conditions, administering medications, and educating patients and families on disease management and prevention. Key Responsibilities: • Provide direct patient care to individuals in their homes, including administering medications, wound care, and health assessments. • Develop and implement individualized care plans based on patient needs and goals. • Monitor patient progress, document care provided, and adjust care plans as necessary. • Educate patients and their families about health management, disease prevention, and self-care techniques. • Communicate regularly with physicians, family members, and other members of the healthcare team to coordinate care. • Ensure compliance with all home health regulations and standards of care. • Participate in ongoing training and professional development to stay current with nursing practices. Requirements: • Registered Nurse (RN) licensure required. • Minimum of 2 years of clinical nursing experience, preferably in Home Health. • Knowledge of home health regulations and standards. • Ability to work independently and manage time effectively in a home setting. • Strong critical thinking and problem-solving skills. • Excellent communication and interpersonal skills. • Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) preferred. • Must have a valid driver's license and reliable transportation. Qualifications: • Graduate of an accredited nursing program. • Proven experience in providing nursing care in a home health setting is preferred. • Ability to assess, plan, and provide patient-centered care. • Ability to work with minimal supervision and make independent decisions. • Strong computer skills, including familiarity with electronic health records (EHR). • Excellent organizational and time-management skills.
