Responsibility

The primary function of the Care Coordinator is to manage, monitor, and update all services and plans in connection with resident care needs through service plans. Analyzes health records and biometric measurements to uncover gaps in care, providing disease-specific education and interventions aimed at filling the identified deficiencies under the supervision of a healthcare provider. Translate policies and protocols to employees, patients, visitors, and government entities as necessary. Operates as the point of contact for guest volume trends related to associate scheduling adherence. In collaboration with the Director of Nursing, the Scheduler is responsible for criticalreal-time decisions regarding increases and reductions and re-allocation of labor. Oversees labor responsibilities including but not limited to monitoring overtime, approving time off, tracking attendance, creating schedules, and adjusting them accordingly. Utilizes inventive and flexible scheduling methods to ensure the schedule is kept current and up-to-date consistently. Participates in recruiting and involving nursing personnel in the hiring process. Works together with the Director of Nursing to manage the distribution of staff tasked with delivering various nursing services, maintaining the appropriate staff-to-patient ratio as required for nursing facilities. Works well with the Care Coordinator to ensure seamless coordination and is willing to work extra shifts every day of the week
