The primary function of the Care Coordinator is to manage, monitor, and update all services and plans in connection with resident care needs through service plans. Responsibilities • Analyzes health records and biometric measurements to uncover gaps in care, providing disease-specific education and interventions aimed at filling the identified deficiencies under the supervision of a healthcare provider. • Translate policies and protocols to employees, patients, visitors, and government entities as necessary. • Operates as the point of contact for guest volume trends related to associate scheduling adherence. In collaboration with the Director of Nursing, the Scheduler is responsible for criticalreal-time decisions regarding increases and reductions and re-allocation of labor. • Oversees labor responsibilities including but not limited to monitoring overtime, approving time off, tracking attendance, creating schedules, and adjusting them accordingly. • Utilizes inventive and flexible scheduling methods to ensure the schedule is kept current and up-to-date consistently. • Participates in recruiting and involving nursing personnel in the hiring process. • Works together with the Director of Nursing to manage the distribution of staff tasked with delivering various nursing services, maintaining the appropriate staff-to-patient ratio as required for nursing facilities. • Works well with the Care Coordinator to ensure seamless coordination and is willing to work extra shifts every day of the week. • Care Coordinators are entitled to a $35 daily bonus for taking on staffing phone duties in collaboration with other team members as needed.
