Responsibility
The primary focus of the position will be providing the JaxCareConnect team with outreach and community health education. Additional priorities include administering Social Determinant of Health screenings, community resource education, managing client evaluation calls, and leading the JaxCareConnect Advisory Council. Engaging in outreach and promoting health education topics and healthcare services. Implementing and executing program using established protocols. Managing program evaluations. Planning and preparing classes. Engaging in program planning. Managing recruitment, planning and implementation for Advisory Council. The supervisor assigns service activities, advises on changes in procedures, and is available for assistance when required. The service activities are reviewed for accuracy through spot checks, through complaints from customers, and through observation of the member at service. The member independently plans and carries out the projects and selects the approaches and methods to be used in solving problems. Completed projects are reviewed very generally to determine that objectives have been met and are in compliance with policies and regulations. Written and oral guides provide specific instructions for doing service. Detailed guidelines exist, including regulations and directives, handbooks, precedents, industry standards, and files of previous projects. Guidelines are generally applicable, but the member independently makes adaptations in dealing with problems and unusual situations. The service position assignments vary significantly or often involve unusual circumstances and incomplete or conflicting data. The member must develop new techniques, methods, or approaches to complete service position assignments

• October 2024 Start* JaxCareConnect Health Literacy and Outreach Coordinator Position Status Open Site Florida NHC Position Type Outreach Coordinator Position Summary • This position will begin in August 2024* JaxCareConnect is a collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents find primary care and a medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to healthcare to support best possible health outcomes. JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.
